Hours: 37.5 per week
Salary: $91,833 to $114,791 annually (To commensurate with experience and consistent with UHN compensation policy)
Shifts: Days, Evenings, Weekends, On Call
Status: Permanent Full-Time

Position Summary

In the pre-admission clinic, the pharmacist is a member of the Surgical Pre-Admission Clinic team, providing patient-focused care to a variety of surgical populations. The pharmacist conducts and documents patient interviews, anticipates and communicates drug therapy problems with inpatient team, and works with interprofessional team to provide medication instructions in advance of elective surgical admissions

The successful candidate must possess sound knowledge in the provision of pharmaceutical care, have superior interprofessional, communication and organizational skills, and demonstrated teamwork in a patient care setting. The candidate must hold a Bachelor of Science in Pharmacy (or equivalent degree) and be eligible for Ontario licensure. Applicants should have experience in providing patient-centered care in a hospital pharmacy setting, and experience in a surgical setting would be an asset. A hospital pharmacy residency certificate, and active membership in CSHP and other professional societies are also assets for this position.

Duties

  • Conducting best possible medication history (BPMH) interviews, performing medication reconciliation, review all doctors' medication orders for appropriateness
  • Identify, resolve and prevent drug therapy problems with the patient and health care team
  • Monitor drug therapy for effectiveness, toxicity and adherence to evidence based medicine/guidelines
  • Follow-up on medication incidents and promote safe medication practices on the unit
  • Respond to drug information questions
  • Patient education
  • Facilitate discharge reconciliation
  • Provide direct and/or functional supervision of technician staff
  • Facilitate and support clinical teaching and research activities within the department, while exemplifying UHN's values of patient needs come first, safety, compassion, teamwork, integrity, and stewardship

Qualifications

  • Possess a Bachelor's of Science Degree in Pharmacy, Doctor of Pharmacy degree or equivalent required
  • Certificate of competency from the Pharmacy Examination Board of Canada required
  • Active registration with the Ontario College of Pharmacists (Part A) required
  • Hospital pharmacy practice experience with patient-centered care experience is a requirement
  • Excellent verbal and written communication skills
  • Excellent customer service and interpersonal skills
  • Excellent organizational and time management skills
  • Hospital Pharmacy residency an asset
  • Membership in the Canadian Society of Hospital Pharmacists and/or other professional societies an asset
