Job Description

Union: Non-Union
Site: Toronto Western Hospital
Department: Pharmacy, Inpatient Unit (patient population may change based on department needs)
Reports to: Pharmacy Clinical Site Leader
Work Model: On-Site
Wage Range: $47.09 - 64.50 per hour (To commensurate with experience and consistent with UHN compensation policy)
Hours: 37.5 Hours per Week
Shifts: Days, Evenings, Weekends, On Call
Status: Permanent Full-Time
Closing Date: March 13, 2025

Position Summary

In this acute and dynamic surgery environment on 9A and 9B, the pharmacist is expected to have highly developed clinical and communication skills as they work closely with the interprofessional team to assess patients, perform medication reconciliation, identify and resolve actual and potential drug therapy problems, review laboratory and diagnostic test results and provide therapeutic drug monitoring. Patient population includes general surgery, bariatric surgery, urology, microsurgery, plastics, ophthalmology, and orthopedic surgery. The pharmacist will work closely with other pharmacists and pharmacy technician staff to ensure the safe delivery and administration of medications to patients.

Duties

  • Conducting best possible medication history (BPMH) interviews, performing medication reconciliation, review all doctors' medication orders for appropriateness
  • Identify, resolve and prevent drug therapy problems with the patient and health care team
  • Monitor drug therapy for effectiveness, toxicity and adherence to evidence based medicine/guidelines
  • Follow-up on medication incidents and promote safe medication practices on the unit
  • Respond to drug information questions
  • Patient education
  • Facilitate discharge reconciliation
  • Provide direct and/or functional supervision of technician staff
  • Facilitate and support clinical teaching and research activities within the department, while exemplifying UHN's values of patient needs come first, safety, compassion, teamwork, integrity, and stewardship

Qualifications

  • Possess a Bachelor's of Science Degree in Pharmacy, Doctor of Pharmacy degree or equivalent required
  • Certificate of competency from the Pharmacy Examination Board of Canada required
  • Active registration with the Ontario College of Pharmacists (Part A) required
  • Hospital pharmacy practice experience with patient-centered care experience is a requirement
  • Excellent verbal and written communication skills
  • Excellent customer service and interpersonal skills
  • Excellent organizational and time management skills
  • Hospital Pharmacy residency an asset
  • Membership in the Canadian Society of Hospital Pharmacists and/or other professional societies an asset
