Job Description

Work Model: On-Site
Wage Range: $47.09 - $64.50 per hour (To commensurate with experience and consistent with UHN compensation policy)
Hours: 0.2 FTE
Shifts: To be confirmed upon hiring
Status: Permanent Part-Time
Closing Date: March 18, 2025

Position Summary
The Pharmacist will be responsible for providing patient-focused care at Toronto Rehabilitation Institute within the Rehabilitation Pain Service. The Rehabilitation Pain Service is one arm of the Comprehensive Integrated Pain Program (CIPP) at UHN and aims to improve and maintain the function of patients with chronic pain. The pharmacist is an integral member of this ambulatory clinic providing pharmacy consultations on medication management.

Duties

  • Provides patient-focused care to improve patient outcomes; this includes identifying, resolving, and preventing drug-related problems by conducting thorough pharmacotherapy assessments, performing medication histories and medication reconciliation, providing drug information to patients and healthcare teams, and other patient care activities
  • Supports operational patient care activities including facilitating medication access and coverage; promotes medication safety
  • Participates in education activities including professional development and continuing education; mentorship and training of new pharmacy staff and learners
  • Participates in development, review, implementation, and evaluation of projects and initiatives in line with the priorities and/or objectives of the pharmacy department and/or organization
  • To provide drug information services to members of the healthcare team to promote the safe, effective, and cost-effective use of medication therapy in the patient care setting
  • Effectively communicates with patients, colleagues, allied health professionals, and other members of the patient-care team
  • Performs other duties consistent with the job classification, as required.

Qualifications

  • Possess a Bachelor of Science degree in Pharmacy, Doctor of Pharmacy degree or equivalent required
  • Certificate of competency from the Pharmacy Examination Board of Canada required
  • Active registration with the Ontario College of Pharmacists (Part A) required
  • Ambulatory pharmacy practice experience preferred
  • Hospital pharmacy residency certificate an asset
  • Membership in the Canadian Society of Healthcare-Systems Pharmacy an asset
  • Excellent verbal and written communication skills
  • Excellent interpersonal skills
  • Excellent organizational and time management skills
