The Toronto Grace Health Centre (TGHC) provides specialized care and services to those individuals who require Complex Continuing Care, Post Acute Care Rehabilitation, Palliative Care, and Transitional Care. The care and services provided at TGHC help facilitate the patient flow through the health care system and ensures recovery to those individuals who may return to their homes or to Long Term Care (LTC) facilities in their communities.

We are looking for a self-motivated and highly engaged Pharmacist to join our Inpatient Pharmacy team. This is a unique and exciting opportunity to work in close proximity with complex patients, caring health care professionals, and hospital staff as we provide services to our growing outpatient patient base and transitional residential beds in the community.

POSITION SUMMARY

Reporting to the Manager, Pharmacy Services, the Pharmacist works according to current Ontario College of Pharmacists standards to promote the safe and appropriate usage of medications and providing clinical monitoring of individual patient drug therapies. This role will also involve operational duties, including procuring, storing, compounding preparations and solutions (in support of the Registered Pharmacy Technicians) and dispensing of drug products and drug information for our patients. The pharmacist will keep abreast of current information and changes in official regulations (including changes to scope of practice) through continuing education and official publications. The pharmacist assumes professional responsibility for the Pharmacy and actively participates in the provision of an essential service for the hospital under the guiding principle of optimizing patient care.

QUALIFICATIONS

Licensed to practice pharmacy in Ontario as Part A Registered Pharmacist (RPh).

Bachelor of Science in Pharmacy (BScPhm) or Doctor of Pharmacy (PharmD).

Knowledge of Provincial and Federal legislation governing all aspects of drug use.

Minimum two years’ relevant experience in hospital pharmacy practice preferred.

Commitment to continuous improvement and learning.

Demonstrated resilience to adapt in an ever-changing environment.

Possess initiative in providing high quality, patient-centered, pharmaceutical care for a diverse and medically complex patient population.

Strong communication and interpersonal skills in order to effectively facilitate the delivery of pharmacy services in a wider institutional setting.

Able to work independently to support safe and efficient pharmaceutical care.

Highly motivated, compassionate individual able to work closely with patients and their family members, physicians, nurses, and other Allied Health professionals using an interprofessional approach to providing patient care.

Knowledge of drug distribution systems, including blister packaging, unit dose, smart medication dispensing devices, e-reminders, etc.

Excellent attendance record.

High degree of accuracy and attention to detail.

Ability to meet bone fide occupational requirements as outlined in the Job Demands Analysis.

MAIN RESPONSIBILTIES

Provide clinical coverage to all units as part of the team supporting Complex Continuing Care, Post Acute Care Rehabilitation, Palliative Care, and Transitional Care and other areas as assigned, based on operational needs.

Works with the Pharmacy Manager in the planning and supporting of pharmacy services in accordance with the Standards of Practice for Pharmacists, and other relevant legislation.

Assists in the orientation of staff and students, provides opportunities for staff/student development and support staff utilization.

Complete assigned workload, and support assignment of workload in the department in conjunction with or in the absence of the Pharmacy Manager.

Practices pharmacy in accordance to the Standards of Practice and relevant legislation such as narcotic and Controlled Drug Act etc.
